Welcome to the OneCare Lab website (“Website”). By accessing this Website, scheduling services, or utilizing any services provided by OneCare Lab, you agree to be bound by the following Terms and Conditions. Please read them carefully.

Services Provided

OneCare Lab provides diagnostic and laboratory-related services, which may include DNA testing, drug testing, rapid urine testing, hair follicle testing, background checks, fingerprinting, and related services. All services are subject to availability and applicable federal, state, and laboratory regulations.

Appointment Scheduling

All appointments must be scheduled in advance through approved channels. Appointment availability is not guaranteed and may be subject to change. Same-day, after-hours, or mobile appointments may incur additional fees.

Identification Requirements

Valid government-issued photo identification is required for all services. Failure to present proper identification may result in cancellation or rescheduling of services without refund.

Payment Terms

Payment is due in full at the time of service unless otherwise agreed to in writing. OneCare Lab accepts approved payment methods only. Services will not be rendered without confirmed payment.

Refund Policy

All sales are final. Due to the nature of diagnostic and laboratory services, refunds are not issued once a service has been initiated, a specimen has been collected, or materials have been prepared.

Rescheduling and Cancellation

Appointments must be canceled or rescheduled at least 24 hours in advance. Late cancellations or no-shows may result in forfeiture of payment and require a new appointment booking.

Client Responsibilities

Clients agree to:

• Provide accurate and complete information

• Follow all instructions provided by staff during service

• Conduct themselves respectfully and cooperatively

• Comply with all identification and documentation requirements

Failure to comply may result in refusal of service and documentation of noncompliance when applicable.

Specimen Collection & Compliance

All specimen collections are performed in accordance with federal, state, and laboratory guidelines. Clients acknowledge that observed collections may be required by regulation, employer policy, or laboratory instruction.

Confidentiality & Privacy

Client information and test results are handled with strict confidentiality in accordance with applicable privacy and data protection laws. Information is released only to authorized parties or as required by law.

Results & Turnaround Times

Test results are provided based on laboratory processing timelines. OneCare Lab does not guarantee turnaround times, as processing is performed by third-party laboratories.

Limitation of Liability

OneCare Lab is not responsible for delays, errors, or outcomes caused by factors beyond its control, including laboratory processing, courier delays, or inaccurate information provided by the client. Services are provided “as is” and without guarantees beyond regulatory requirements.

Website Use

Use of this Website is for informational purposes only. Unauthorized use, misuse, or attempt to interfere with the Website’s functionality is prohibited.

Third-Party Services

OneCare Lab may utilize third-party laboratories, vendors, or service providers to complete services. OneCare Lab is not responsible for the policies or actions of third parties beyond reasonable due diligence.
